Achalasia is a chronic, idiopathic disease characterized by
primary motility disorder of the esophagus. There is no peristalsis in the esophagus after swallowing.
There is either no or very little relaxation in the lower esophageal sphincter. Distal esophageal spasm is
an idiopathic esophageal motility
disorder manifested by difficulty swallowing and chest pain.
Although the etiology is not fully understood, studies have shown that tissue NO deficiency
disrupts inhibitory innervation, causes simultaneous contractions and abnormal relaxations at the
esophagocardiac junction. High-
resolution manometry is the gold standard diagnostic method
for the diagnosis of achalasia and distal esophageal spasm. Laparoscopic surgical myotomy and
anterior fundoplication are the most effective surgical treatment methods.
